This motor is the older 12 volt motor for SAM pool tables using the Dual Mech PCB, fitted to tables prior to 2003.
If your SAM table has a PCB mounted on the back wall of the cash area, facing out, and has a smaller PCB next to it on the left then you have a 12 volt system.

For example, no deliveries before or after specific times, leave with a neighbour etc. Please also provide a contact phone number as our carriers will use this in the event of any problems in locating your address or finding anyone to sign for delivery. The more information we have the better service we can provide.
